Therefore, each patient is paired with an interdisciplinary team who collaborates with them and their family to create a tailored care plan based on need of symptom management and goals of care.

The Solace Palliative Care program is provided at no cost to patients or their families, reflecting Solace’s commitment to addressing the healthcare needs of the local community.

Our Team
The Solace Palliative Care team includes a nurse for monthly visits and symptom management support, a social worker to provide support and connect patients and families with community resources, and a palliative team coordinator to oversee the care coordination. The team also holds periodic meetings to assess the status and needs of all palliative patients, ensuring the best possible care and outcomes.
